29 CFR 1926.451(c)(2)(ii): Unstable objects shall not be used to support scaffolds or platform units.  A) Exterior, Landing above the front door:  Employees were exposed to fall/scaffold collapse hazards of approximately 17 feet while working on a wooden landing that was being used as a make shift scaffold and was not erected on firm foundation, on or about March 3rd, 2015.

29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees: without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph.Note:  This paragraph does not apply to large area scaffolds where the employer has satisfied the following criteria: the ladder was placed against a structure which was not a part of the scaffold and the scaffold was secured.   A) Upper landing of the structure on landing: An employee was exposed to fall/ladder collapse hazards of approximately 16 feet as the employee was using a folding step ladder to increase the working level height of the scaffold, on or about March 3rd, 2015.
