29 CFR 1926.403(i)(2)(i): Except as required or permitted elsewhere in this subpart, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, or by any of the following means: (a) Dusit Thani Guam Resort Hotel, Ground Level: On or about April 15, 2015, a wall mounted receptacle cover was not provided when an employee plugged in a portable fluorescent light stand, exposing the employee to the hazards of possible electrical shock and burns.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, and enclosures was not permanent and continuous: (a) Dusit Thani Guam Resort Hotel, Ground Level: On about April 15, 2015, there was no grounding prong on a cord that powered a portable fluorescent light stand. The cord was plugged into a wall mounted outlet receptacle 110 VAC. One painter was exposed to the hazards of electrical shock.
