29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c):       a) Southwestern Bell Telephone Company, DBA AT&T Missouri @ Newport Landing & Hawkins Road, Fenton, MO 63026: On and before April 10, 2015, the employer did not ensure that employees were protected from cave-ins by a protective system in that the system that was installed did not protect the entire height of the west wall of the excavation. The excavation measured approximately 5 feet in depth along the east wall and approximately 7 feet in depth along the west wall. The upper portion of the west wall, approximately 33 inches, was unprotected. This condition exposed two employees to struck-by hazards.     b) Southwestern Bell Telephone Company, DBA AT&T Missouri @ Newport Landing & Hawkins Road, Fenton, MO 63026: On and before April 10, 2015, the employer did not ensure that employees were protected from cave-ins by a protective system in that ladder access for the excavation was located in an area that was unprotected. The excavation measured approximately 5 feet in depth along the east wall and approximately 7 feet in depth along the west wall. This condition exposed employees to struck-by hazards.
