29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: (a) On or about 04/12/2015 - 1735 Childhaven Road, Cullman, AL, employee(s) removing and replacing a metal roof where the holes were not protected with effective fall protection guarding were exposed to falls of 17 feet to the concrete floor below.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person: (a) On or about 04/12/2015 - 1735 Childhaven Road, Cullman, AL, the Guardian Ridge-It Safu Anchor was not installed properly in order to meet the manufacturer's 5,000 lb. requirement. Only two to three screws were used instead of the required eight screws and the anchor ends were bent over to allow attachment of the SRL resulting in forces exerted vertically on the existing screws.
