An employee was struck-by a forklift while checking special order tires for accuracy near the loading dock. Employees were exposed to struck-by and amputation hazards.

29 CFR 1910.136(a): The employer did not ensure that each affected employee uses protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, and where such employee's feet are exposed to electrical hazards: 1) Warehouse Area - The employer failed to ensure employees working in an environment around powered industrial trucks were provided with adequate personal protective equipment such as but not limited to steel toe shoes. An employee was struck-by a powered industrial truck and sustained an amputation to the big toe.
