(b)   Averitt Express - On or about April 16, 2015 employees were exposed to a struck-by hazard and possible vehicle accidents due to the fact several vehicles had worn tires.      (c)   Truck 160 & 158 - On or about April 16, 2015 employees were exposed to a struck-by hazard and possible vehicle accidents due to vehicles having worn wiper blades.      (d)   Truck 143 & 158 - On or about April 16, 2015 employees were exposed to a struck-by hazard and possible vehicle accidents due to vehicles having at least one brake light out.  These vehicles are driven at night for a 24 hour operation.       (e)    Truck 143 - On or about April 16, 2015 employees were exposed to a struck-by hazard and possible vehicle accidents due to vehicle only having one operational head lamp. This vehicle is driven at night for a 24 hour operation.      (f)     Truck 144 - On or about April 16, 2015 an employee was exposed to a struck-by hazard due to this vehicle not having an operational seat belt installed. Both seat belt buckles were removed.
