On or about April 13, 2015, an employee was injured when removing masking materials while the Hobart baggage loader was being run and there was no hazard assessment conducted for nonroutine tasks.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: (a) Grinding Areas and Paint Booth: Employees were exposed to paint vapors and dusts. Employees were required to wear either N95 3M Dual Cartridge 1/2 faced respirators or 3M Particulate Respirator N95, when grinding, and/or painting. There was no written respiratory protection program to ensure the proper use of respirators and to establish worksite specific procedures when using respiratory protection.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Detailing Bay: An employee was exposed to amputation hazards from rotating parts and ingoing nip points from the Hobart-Wollard TC886 Gas Aviation Belt Loader near the front portion of the underneath side of the conveyor. On or about April 13, 2015 an employee was injured when two of his fingers got caught between the pinch point of a roller and the conveyor belt when removing paper and tape used to mask the underside of the conveyor during painting.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: (a) Employees were exposed to hazardous chemicals such as, but not limited to, Sherwin-Williams Dimension Mixing Color, Magenta containing Toluene and Xylene which may cause nervous system depression, and Dupli-Color Flexible Bumper Coating Gray that contains Propane and Butane, which is a flammable and could cause eye, skin and respiratory system irritation. There was no written hazard communication program describing how chemicals were to be labeled, safety data sheets were to be kept, and how employees were to be trained on using hazardous chemicals.
