29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  a) Throughout Facility - Employer failed to provide the respiratory protection program to employees required to wear the NIOSH 3M Dual Cartridges respirators when painting and prepping the paints for automobiles.  Employees were working with lacquer thinners, primers and activators that include ingredients such as Isocyanates. Employees were exposed to respiratory tract irritations.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  a) Throughout facility - Employer failed to implement and provide a written hazard communication program and information for GHS to employees required to work with hazardous chemicals such as, but not limited t,o 2K Activator, 2K Urethane Activator, Basecoat Binder and Smart Lightweight Body Filler that included ingredients such as Isocyanate and Styrene. Employees were exposed to respiratory and skin irritations, dermatitis and skin corrosion.

Employees were exposed to skin irritations and dermatitis.

The last annual inspection of fire extinguishers was performed on July 21, 2014. Employees were exposed to fire hazards.
