29 CFR 1910.95(b)(1): When employees were subjected to sound exceeding those listed in Table G-16, feasible administrative or engineering controls were not utilized: An employee operating press number 2003 was exposed to continuous noise levels at 167.5% of the permissible daily exposure (8-hour, time weighted average sound level of 90 dBA or equivalently, a dose of 100 percent), during the 415 minute sampling period on 4/17/2015; exposure calculations included a zero increment for the 65 minutes not sampled. A TWA of 93.72 dBA.

29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: a) An employee operating press number 2003 was exposed to continuous noise at 177% of the permissible daily noise exposure (8-hour time-weighted average sound level of 90 dBA) during the 415 minute sampling period on 4/17/2015; exposure calculations included a zero increment for the 65 minutes not sampled. b) An employee operating press number 3003 was exposed to continuous noise at 97.7% of the permissible daily noise exposure (8-hour time-weighted average sound level of 90 dBA) during the 387 minute sampling period on 4/17/2015; exposure calculations included a zero increment for the 93 minutes not sampled. c) An employee operating presses number 5000 and 1505 was exposed to continuous noise at 87.4% of the permissible daily noise exposure (8-hour time-weighted average sound level of 90 dBA) during the 418 minute sampling period on 4/17/2015; exposure calculations included a zero increment for the 62 minutes not sampled. d) An employee operating press number 1500 was exposed to continuous noise at 70.7% of the permissible daily noise exposure (8-hour time-weighted average sound level of 90 dBA) during the 413 minute sampling period on 4/17/2015; exposure calculations included a zero increment for the 67 minutes not sampled. e) An employee operating press number 3004 was exposed to continuous noise at 71.4% of the permissible daily noise exposure (8-hour time-weighted average sound level of 90 dBA) during the 400 minute sampling period on 4/17/2015; exposure calculations included a zero increment for the 80 minutes not sampled.
