29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    On or about April 18, 2018, the employer failed to review their written lock out tag out  procedures at least annually to ensure that it was up to date , effective and being followed properly.  Employees were exposed to a struck by and/or caught in between hazard while performing the task of clearing a jam on the #5001 (Serial #S2-20770) 500 ton automated press.  Employees were not trained to perform lock out tag out nor were they applying the lock out tag out procedure to this task.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment:    On or about April 18, 2018, the employer failed to ensure that the automated press #501 (serial #S2-20770) was locked and tagged out per the employers written procedure prior to employees attempting to clear a jam (the material had buckled up inside the press).  Employees are clearing jams without locking and tagging out the press and are exposed to struck by and/or caught in between hazards.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:    On or about April 18, 2018, the employer failed to ensure that employees who were tasked with job duties, to include but not limited to the clearing of jams on the automated presses, were trained as authorized to perform lock out tag out.  Employees are clearing jams, not applying the employers lock out tag out procedure and are exposed to struck by and/or caught in between hazards on the presses, including but not limited to:    a)  Press #3001 (serial #SE4-28378).    b)  Press #5001 (serial #S2-20770).    c)  Press #8001 (serial #S2-27983).    NOTE:  The employer was previously cited for a violation of this occupational safety and health standard or it's equivalent standard which was contained in OSHA Inspection #1047617, citation 1, item 1, which was affirmed as final order on November 16, 2015, with respect to the workplace located at 20303 First Avenue, Middleburg Heights OH 44130.

29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press (es):    On or about April 18, 2018, the employer failed to ensure that employees were protected from a struck by and/or caught in between hazard after re-initiating the start up operation of a press after employees had completed the task of clearing a jam on an automated press (press #5001, serial #S2-20770).      NOTE:  The employer was previously cited for a violation of this occupational safety and health standard or it's equivalent standard which was contained in OSHA Inspection #1047617, citation 1, item 6, which was affirmed as final order on November 16, 2015, with respect to the workplace located at 20303 First Avenue, Middleburg Heights OH 44130.
