29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:    a) Second Floor: CSHO observe an employee exposed to a fall hazard of approximately 8 feet as he slid down a wall brace while engaged in framing activities, on or about 3/28/2015.

29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact.      a) Ceiling Level Second Floor: Employees without any eye protection were exposed to eye injury while engaged in framing activities using  pneumatic nail guns, on or about 3/28/2015.    b) Ground Level - Right: Employee without any head protection was exposed to head injury while carrying ceiling joists bellow other employees engaged in framing activities, on or about 3/28/2015.    A & R Quality Construction Corp was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.95(a)which was contained in OSHA inspection number 950192, citation number 01, item number 01 and was affirmed as a final order on 4/3/2014, with respect to a workplace located at 32 Alan Ave., Glen Rock, NJ.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       a) Ceiling Level  Second Floor: Employees without any means of fall protection were exposed to fall hazards of approximately 18 feet while engaged in framing activities, on or about 3/28/2015.             A & R Quality Construction Corp was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 950192, citation number 01, item number 02 and was affirmed as a final order on 4/3/2014, with respect to a workplace located at 32 Alan Ave., Glen Rock, NJ.
