On or about 4/16/2015, in the Melt Shop, the employer required employees working on the 3rd level and above to wear respirators with tight-fitting face pieces, but did not ensure that all employees had received fit testing. 2.) On or about 4/16/2015, in the Melt Shop, the employer required employees working on the 3rd level and above to wear respirators with tight-fitting face pieces, but did not ensure that employees were fit tested for all facepieces worn by employees.
