29 CFR 1926.652(c)(2)(ii): Deviation from the specifications, recommendations, and limitations issued or made by the manufacturer were allowed without the manufacturer's specific written approval. (a) Speer Bros., Inc. on or about April 15, 2015, at the site located near the intersection of 2nd and Jackson St. in Port Clinton, OH: The employer failed to attain specific written approval from the manufacture of the GME 6M820 trench shield for the addition of steel plates which extend the height of the shield by 7ft. This addition/modification to the engineered system adversely affects the manufactured shield by exerting added side wall pressure in excess of the manufactures designed load rating. An employee working in this condition was exposed to crushing hazards.
