Erie St. Lorain OH 44052, The employer . did not ensure that an employee was protected from head impact hazards while working below ground level in a trench. Employees installing 16 inches waterline in a 6 feet deep trench without the benefit of head protection, was exposed to potential struck by hazards unshielded soil, overhead equipment operations, and tools.
