29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On the 5-12 pitch roof of a multifamily residential property, employees were not protected from falling while installing plywood sheeting, exposing the employees to a 19 foot fall hazard, on or about 4/15/15.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a. Against a 2nd floor interior wall, the portable ladder used to gain access to the roof trusses did not extend 3 feet past the upper landing surface, exposing employees to an 8 foot fall hazard, on or about 4/15/2015.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:  a. Against a 2nd story interior wall, the portable ladder used to gain access to the roof and truss work was not secured from displacement, exposing employees to an 8 foot fall hazard, on or about 4/15/2015.
