29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: Apartment building under construction, east side of Unit A2, JLG Aerial Lift (Model: L20-8.2K Serial: 650457), on or about April 15, 2015, employees were exposed to a fall hazard of approximately 32 feet to the unpaved ground below, when working in a aerial lift without wearing a harness while not being trained on the procedures to recognize fall hazards.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: Apartment building under construction; East Side of Apartment Unit A2, JLG Aerial Lift (Model: L20-8.2K Serial: 650457), On or about April 15, 2015, employee(s) were exposed to a fall approximately 32 feet in height to unpaved ground level below, while installing fascia siding on a apartment complex from an aerial lift without wearing a harness. Francisco Guzman was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.453(b)(2)(v), which was contained in OSHA inspection number 1021404, citation number 1, item number 1 and was affirmed as a final order on 2/18/2015, with respect to a workplace located at 1210 North Hunt Lane, San Antonio, TX 78251. Francisco Guzman was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.453(b)(2)(v), which was contained in OSHA inspection number 1004840, citation number 1, item number 1 and was affirmed as a final order on 2/18/2015, with respect to a workplace located at 7035 Pickwell Dr., San Antonio, TX 78214.
