29 CFR 1910.28(a)(1): Scaffolds were not furnished or erected in accordance with this standard for persons engaged in work that could not be done safely from the ground or from solid construction:  On or about April 3, 2015, VCNA Prairie, Inc. did not provide a scaffold for maintenance employee use within the inwardly converging walls of the mixer dump hopper.  Employees were exposed to a fall hazard of at least 14 feet through the discharge of the mixer dump hopper.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
