29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition. Employees working from the catwalk at the top of a material bin were exposed to slip, trip, and fall hazards when the employer failed to ensure the area was kept clean and orderly and in a sanitary condition. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. Employees performing maintenance operations were exposed to a 10 foot fall while traversing the second floor work platforms on both sides of the bin to conduct work activities and the employer failed to ensure the platform was guarded. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.23(c)(3): Regardless of height, open-sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards shall be guarded with a standard railing and toe board. Employees were exposed to engulfment hazards from falling into a material bin while clearing an overflow sensor and the employer failed to ensure a standard railing system was installed that consisted of a top rail with a vertical height of 42" and a mid-rail. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
