The cable splice was located 48 above the work surface, and not otherwise protected from damage.    -------------------------------------------------------------------------------------------------------------------------------------------            29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:    a) Northeast 2nd Floor Hotel Wing: A 12/2 NM (non-metallic) cable was wired directly into a cord cap (male plug end) which was drawing power from a duplex electrical outlet.  NM cable is listed and labeled to be used for branch circuits into electrical boxes not into reloadable service plugs, thereby exposing employees to an electrical hazard on or about April 16, 2015.    b) Southeast 2nd Floor Hotel Wing: A 12/2 NM (non-metallic) cable was wired directly into a cord cap (male plug end) which was drawing power from a duplex electrical outlet.  NM cable is listed and labeled to be used for branch circuits into electrical boxes not into reloadable service plugs, thereby exposing employees to an electrical hazard on or about April 16, 2015.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.405(g)(2)(iii): Flexible cords were not used in continuous lengths without splice or tap:  a)  NORTHEAST HOTEL WING, 3RD FLOOR STAIRWELL LANDING:  On or about April 16, 2015 a 12/2 NM (non-metallic) cable was spliced with a 14/2 minimum duty flat (flexible) cord exposing employees to shock hazards. The 14/2 flat cord splice located 48" above the working surface is not used in a continuous length, does not retain the insulation, outer sheath properties and usage characteristics to be used in this manner.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
