29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition when it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact: 1520 HARRISBURG PIKE, LANCASTER, PA, CONESTOGA 1ST FLOOR CORRIDOR - Employee(s) performing work on live energized electrical wires did not use personal protective equipment, specifically gloves rated for electrical use, that would have provided protection from electrical shock injuries from incidental contact with the live circuits. An employee stripping a live 277 volt electrical conductor made contact with the energized wire and sustained an electrical shock injury, on or about May 8, 2015. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: (a) 2420 GEHMAN LANE, LANCASTER, PA - The employer failed to notify OSHA within the 24 hour time period to report a hospitalization of an employee, on or about May 15, 2015. No abatement certification or documentation required.
