29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): Location: 83 Pacific Street Rockland MA Instance 1: Employees were exposed to falls of approximately 13 feet while shingling the roof of the front porch. Instance 2: An employee was exposed to falls of approximately 30 feet while receiving shingles from a ladder hoist while standing on the roof. The Tony Roofing Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501 (b) (13), which was contained in OSHA inspection numbers , 314918533 Citation item number 2-1 and was affirmed as a final order on 9/1/11, with respect to a workplace located at 1272 West Main Road Middletown RI.

Location: 83 Pacific Street, MA An employee was exposed to a fall hazard while using an 20 foot aluminum extension ladder that was not at the proper angle.
