29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, shall be kept cleared from work areas, passageways, and stairs, in and around buildings or other structures. (a) Single Level Concrete Building, Outside: On or about April 20, 2015, employer did not prevent concrete debris, cement blocks, and metal pipes scattered about working and walking areas around exterior of the building. The employees were exposed to hazards of tripping and falling, possibly resulting in sprains, fractures, and even concussions.

29 CFR 1926.28(a): The employer is responsible for requiring the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions or where this part indicates the need for using such equipment to reduce the hazards to the employees. (a) Single Level Concrete Building, Kitchen Area: On or about April 20, 2015, one worker breaking a concrete table was not wearing appropriate eye protection while using a Makita Demolition Hammer for heavy chiseling and chipping away old concrete table. The worker was exposed to possible eye related injuries.
