29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: a) On April 15, 2015 and times prior, the competent person did not conduct an adequate inspection of the trench prior to start of work to make sure that employees were protected from cave-in hazards. The east side of the trench wall was vertical exposing employees working in that area of the trench to a cave-in hazard. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet Letter").
