29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: 1) On July 20, 2015, an employee was exposed to cave in hazards while working in multiple trenches that did not have adequate protective systems. The competent person had not conducted inspections of the trenches as needed to ensure employees were not exposed to cave in hazards.
