29 CFR 1910.132(c): Personal protective equipment was not of a safe design and construction for the work performed: a) On or about April 21, 2015, at the above located workplace, an employee was required to work 15 feet or more above the floor using a body belt with an energy absorbing lanyard which was approximately six feet long. The employer did not protect the employee by the use of an approved personal fall arrest systems. Employee was thereby exposed to the hazards associated with falls.
