29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    (a)  On or about January 19, 2015, the employer did not ensure employees were safe while using an Avanti Hytorc hydraulic torque/tension tool by training the proper use and hazard recognition and mitigation prior to assigning the employees to use the tool.  An employee's fingers were pinched and required medical attention when the machine was activated and caught the fingers against a pipe.      Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
