29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: On or about June 30, 2015 the employer did not ensure that all employees working on the RTA Expansion Project occurring adjacent to North Rampart Street in New Orleans, Louisiana, likely to be exposed to electrical shock hazards from underground utility lines were adequately trained to recognize the presence of such underground lines when marked and how to avoid contact with these lines during the course of their work.

29 CFR 1926.416(a)(2): In work areas where the exact location of underground electric power lines was unknown, employees using jackhammers, bars, or other hand tools which may contact a line were not provided with insulated protective gloves: On or about June 30, 2015 the employer did not ensure that employees performing concrete chipping operations using a DeWalt SDS Hammer Drill were adequately protected from possibly electrical shock and/or electrical burn resulting from contact with underground electrical power lines that were not appropriately discovered and marked by means of insulated protective gloves.
