29 CFR 1926.1053(b)(1): When a portable ladder was used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access: On the roof of a commercial building, west side, on or about April 21, 2015, employees were exposed to fall hazards of approximately 11 feet to the paved ground below, when using a ladder that was not 3 feet above the upper landing surface, while gaining access to a roof.
