29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee working from an aerial lift. On or about April 21, 2015, the employer failed to require the employee wear a body belt attached to the boom basket while operating a Grove boom lift during the installation of windows at the job site located at 4917 Canfield Road, Canfield Ohio 44066.
