29 CFR 1926.403(i)(2)(i): Except as required or permitted elsewhere in this subpart, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by cabinets or other forms of enclosures, or by any of the following means:    a.  On or about April 21, 2015 - at the above addressed jobsite, employees and subcontractor employees were walking/working around exposed live electrical wires hanging from ceiling lamps and the old ceiling grid, thereby exposing the employees to electrical hazards.    b.  On or about April 21, 2015 - at the above addressed jobsite, employees and subcontractor employees were plugging in electrical equipment/tools in to live exposed electrical outlets that had damaged covers or face plates to protect against accidental contact, thereby exposing the employees to electrical hazards.

29 CFR 1926.405(b)(2): Covers and canopies. All pull boxes, junction boxes, and fittings shall be provided with covers. If metal covers are used, they shall be grounded. In energized installations each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  a.  On or about April 21, 2015 - at the above addressed jobsite, a live electrical panel was not protected with a cover, thereby exposing employees to arc flash/blast hazards.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a.  On or about April 21, 2015 - at the above addressed jobsite, an employee performing carpentry operations from a mezzanine, approximately ten (10) feet three (3) inches above the ground was not protected from falls, thereby exposing the employee to fall hazards.
