All pull boxes, junction boxes, and fittings were not provided with covers:             a)  On or about March 21, 2016, on the first floor, in the rear, of the above located jobsite, two 240 volt live electrical panels were not protected with covers. Employees working at the jobsite were thereby  exposed to arc flash/blast hazards.          b) On or about March 21, 2016, on the second floor, in the front, of the above located jobsite, two 480 volt live electrical panels were not protected with covers. Employees working at the jobsite were thereby  exposed to arc flash/blast hazards.          The Stack Development, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.405(b)(2), which was contained in OSHA inspection number 1057605, citation number 1, item number 2b and was affirmed as a final order on July 15, 2015, with respect to a workplace located at 5900 W. 159th Street, Oak Forest, Illinois.
