29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and, that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user    a) Facility - on or about 4/23/15, employee(s) had been provided 3M, half face elastomeric respirator(s) to work with Avesta RedOne (hydrofluoric acid, nitric acid) while cleaning stainless steel domes.  The employer had not implemented including those elements of a respiratory protection program that were necessary to ensure that any employee using a respirator was medically able to use that respirator, and that the respirator was cleaned, stored and maintained so that its use does not present a health risk to the user.     b.) Facility - on or about 4/23/15, employee(s) had been provided half face elastomeric respirators to perform certain welding operations such as but not limited to welding on galvanized metal. The employer had not implemented including those elements of a respiratory protection program that were necessary to ensure that any employee using a respirator was medically able to use that respirator, and that the respiratory was cleaned, stored and maintained so that its use does not present a health risk to the user.     ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    a.) Facility - On or about 4/1/15, employees were using Avesta RedOne Pickling Paste140 (hydrofluoric acid, nitric acid/corrosive) to completely clean stainless steel domes.  The employer had not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area.     b.) Facility - On or about 4/23/15, employees use Avesta Blue One (hydrofluoric acid, nitric acid, sulphuric acid/corrosive) and Neutralizing Agent 502 (sodium hydroxide/corrosive) to clean metal welds. The employer had not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area.     ABATEMENT DOCUMENTATION REQUIRED
