On or about April 23, 2015, the employer failed to ensure employees cleaning chips from the Phoenix CTC Copying Milling Machine, model number VTC-30-10, serial number 131056, placed a locking device on the equipment to ensure no accidental startup.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about April 23, 2015, the employer failed to ensure employees operating the AmeraSeiki CNC, model number VB-2, serial number 2010028 were protected against accidental contact with the rotating head. The interlocks are inoperative. The Canton Drop Forge was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 315443770, citation number 02, item number 003 and was affirmed as a final order on November 14, 2011, with respect to a workplace located at 4575 Southway SW. Canton, Ohio 44706. The Canton Drop Forge was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 315552166, citation number 02, item number 001 and was affirmed as a final order on July 2, 2012, with respect to a workplace located at 4575 Southway SW. Canton, Ohio 44706.
