29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.     On April 22, 2015, an employee was exposed to a 13.3 foot fall while walking/working on the second floor of a commercial building and the employer did not ensure that employees utilized fall protection.     Phoenix Builders, LTD. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11) which was contained in OSHA inspection number 107766, citation number 01, item number 01 and was affirmed as a final order on January 28, 2012, with respect to a workplace located at  Manorcare Health Services, Homewood, IL 60422.     Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
