29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     On or about February 25, 2020, employees utilized an extension ladder that did not extend three feet past the upper landing they accessed, exposing employees to a fall hazards of approximately 19 feet to the ground below.         Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.     On or about February 25, 2020, employees walking/ working from a commercial roof were not protected from a fall, exposing employees to fall hazards of approximately 19 feet.     PHOENIX BUILDERS, LTD.  was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1058069, citation number 2, item number 1 and was affirmed as a final order on June 6, 2015, with respect to a workplace located at 6501 Cass ave. Westmont, IL 60559.     Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
