29 CFR 1917.112(b)(1): Guardrails were not provided at locations where employees were exposed to floor or wall openings or waterside edges, including bridges or gangway-like structures leading to pilings or vessel mooring or berthing installations, which presented a hazard of falling more than 4 feet or into the water. (a) Belt galley: On or about April 21, 2015 and times prior thereto, multiple platforms did not have a guard rail or self closing gate. This exposed employees to a 20 to 40 foot fall. (b) Walkways top of bins: On or about April 21, 2015 and times prior thereto, there were multiple ladder openings with no guardrail or swing gates to protect employees. This exposed employees to over 10 foot falls.

29 CFR 1917.112(c)(3): The top surface of guardrails installed after October 3, 1983, was not 42 inches, plus or minus 2 inches high: Conveyor to spout: On or about April 21, 2015 and times prior thereto, the railing going out to the spout was at a height of 34 inches. This exposed employees to potential falls over the rail from being to low.

Truck hoist winch tower: On or about April 21, 2015 and times prior thereto, the ladder was 43 feet and did not have any safety features. This exposed employees to a potential fall of up to 43 feet.
