This condition exposed employees to caught-in hazards.  (b)  Hogue Warner Elevator, Barge Conveyor:  On or about April 21, 2015 and at times prior thereto, a seven (7) foot section of conveyor rollers and belt directly below the Hi-Roller motor and adjacent to the catwalk was not guarded.  This condition exposed employees to caught-in hazards.

This condition exposed employees to fire and smoke inhalation hazards from the inability to quickly extinguish an incipient stage fire.
