(a)  Top platform:  On or about April 21, 2015 and times prior thereto, there was an area that did not have a guard rail and had a gap that employee could fall through. This exposed employees to a 8 foot fall to the lower platform.  (b)  Top of bins:  On or about April 21, 2015 and times prior thereto, the crossover at the drag chain railing was not adequate that if a person tripped or fell forward would protect them.  This exposed employees to a potential 70 foot fall to the ground.
