29 CFR 1910.272(f)(2): The permit did not certify that the requirements contained in 29 CFR 1910.252(a) had been implemented prior to beginning the hot work operations in the grain handling facility: Boardman Terminal: On or about April 21, 2015 and at times prior thereto, where hotwork permits were issued, the employer did not ensure that portable fire extinguishers were maintained in a state of readiness, including monthly inspections of such equipment. This condition exposed employees to fire and smoke inhalation hazards from the inability to quickly extinguish an incipient stage fire.
