OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:    a. On or about 04/23/2015 at the above addressed jobsite, a supervisor was attempting to empty an over-filled trash compacter container into the back of his Recycling Collection Vehicle (Trash/Recycle Truck), wherein the crews was not trained to operate the vehicle, and wherein the employer had not developed and implemented a job task procedure, and wherein the vehicles Lock Bar was defective, and wherein the trash container safety/locking pins where not installed, and, therefore allowing the container door to abruptly open when the container was rotated upside down, which subsequently resulted in the supervisor being struck in the head by the steel door, and to which the supervisor received a serious head injury/hospitalization.         NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
