29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a. 3rd Floor - Employees and subcontractors were not protected from falls of approximately 10 feet when a wall opening next to the rear stairwell was left unguarded.

29 CFR 1926.1052(b)(1): Foot traffic was not prohibited on stairways where the treads and/or landings of pan stairs were to be filled in with concrete or other material at a later date and stairs were not temporarily fitted with wood or other solid material at least to the top edge of each pan:  a. Front Stairwell - Employees and subcontractors were not protected from tripping hazards when pan stairs that were yet to be filled in were used to access the second and third floors and were not fully supplied with a temporary solid material.   b. Rear Stairwell - Employees and subcontractors were not protected from tripping hazards when pan stairs that were yet to be filled in were used to access the second and third floors and were not fully supplied with a temporary solid material.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed:   a. Roof Hatch - Employees and subcontractors accessing the roof through a hatch were not protected from falls of approximately 12 feet when a single section of an aluminum extension ladder only reached the base of the hatch, requiring employees and subcontractors to use the roof joists as footing.
