North building - On or about November 10, 2015, the employee assigned signal person responsibilities was not trained to recognize the hazards relating to crane operations or the procedures to follow to minimize the hazards.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a. North building - On or about November 10, 2015, employees accessing the third-floor work area through a floor opening were not protected from falls of approximately 10 feet when a 5 foot stepladder was in use and it required employees to use the interior frame walls and floor joists as footing while accessing and exiting the third-floor level.  Clint Forrest d/b/a CFA Builders was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1058529, citation number 1, item number 3, and was affirmed as a final order on 06/02/2015, with respect to a workplace located at 49 Main St. Durham, NH.

North building - On or about November 10, 2015, fall protection was not utilized by employees performing framing activities on the third-floor apartment building while exposed to falls of over 21 feet.  Clint Forrest d/b/a CFA Builders was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1058529, citation number 1, item number 1, and was affirmed as a final order on 06/02/2015, with respect to a workplace located at 49 Main St. Durham, NH.
