On April 20, 2015, at the gravel pit across from the stadium, the backhoe operator was not wearing the seat belt while operating the equipment on uneven surface and rough terrain, exposing the operator to struck-by and caught between hazards.

On April 20, 2015, at the grave pit across from the stadium, at least one heavy equipment operator was not required to wear safety shoes. The operator was observed around heavy equipment on uneven surface wearing slipper sandals.
