29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: (a) Front side of House, during masonry operations, employees, who were working at the jobsite with Makita skill saw and other equipment requiring electrical equipment power were exposed to electrocution, and electrical shock hazards from a 120 volt powered extension cord plugged into the jobsite electrical receptacle that had the power cord protective sleeve pulled out of the crimped clamp exposing the internal wiring. The power cord did not have a strain relief mechanism.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access: (a) At the residential home (Tubular Welded Scaffold), during masonry operations, an employee, who was climbing 13-feet on crossbraces from the lower surface level to access the second tier of the south side tubular welded scaffold. The employer did not provide a ladder or other means of direct access to the second tier of the scaffold and an employee were exposed to the hazards of falling to the ground below.
