29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. On April 18, 2015 at the workplace located at 1101 N. Redmond Road, Jacksonville, Arkansas, an employee sustained an injury resulting in hospitalization. The employer failed to report to OSHA the work-related in-patient hospitalization within twenty-four hours as required.
