29 CFR 1910.212(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to broken pieces of pipe or high velocity streams of water sufficient to cause death or serious physical harm which could be ejected if a pipe bursts when under pressure during the testing procedure: a- Finishing Department, Small Test Press. On or about 4/24/15, a pipe ruptured during the pressure testing procedure. There was a 10" gap in the expanded metal mesh guarding through which projectiles or high velocity streams of water streams of water sufficient to cause death or serious physical harm from pipe being tested under pressure could be ejected. Condition noted on or about 4/24/15.
