29 CFR 1910.22(a)(2): The floor of every workroom was not maintained in a clean and, so far as possible, a dry condition. Where wet processes are used, drainage shall be maintained, and false floors, platforms, mats, or other dry standing places should be provided where practicable.  This violation was observed on or about April 28, 2015, in areas such as, but not limited to the area in the door shop around the TS-120 Sander, explosive dust was on the floor, walls, and around the machines where sanding operations were performed.

29 CFR 1910.37(b)(2): Each exit must be clearly visible and marked by a sign reading "Exit."  This violation was observed on or about April 28, 2015 at the following locations:  a) The exit door on the North wall of the door shop did not have a functioning  Exit sign.     b) The exit door on the east wall of the panel department building area did not have an Exit sign.   c)  The exit door on the west wall of the panel department did not have a functioning Exit sign.      Employees were exposed to the hazards of delay in exiting the building in case of an emergency.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  This violation occurred on or about April 28, 2015, the employer did not develop a written energy control (lockout/tagout) program to include, but not limited to, an energy control procedure, employee training, and periodic inspection of the specific written lockout/tagout procedures exposing employees to the hazards of unexpected energization of equipment.

29 CFR 1910.213(d)(1): Each circular crosscut table saw was not guarded by a hood which met all the requirements of paragraph (c)(1) of this section for hoods for circular ripsaws.   a)  This violation was observed on or about April 28, 2015, the saw blade on the Delta Unisaw in the specialty shop was not guarded exposing employees to the hazards of being struck by the sawblade.      b)  This violation was observed on or about April 28, 2015, the saw blade on the Delta Unisaw on the east wall of the panel department was cracked/broken exposing employees to the hazards of being struck by the sawblade.

These guards shall effectively prevent the hands or fingers of the operator from coming in contact with the nip points. This violation was observed on or about April 28, 2015, in such as, but not limited, to the door area:                  a)      The duplex outlet box behind the TS-120 Sander, which is missing faceplate attachment screws, is not approved for a Class II Division 2 location exposing employees to the hazards of dust explosion.       b)      A flexible cord connector was missing from the opening of the junction box exposing employees to the hazards of dust explosions.
