29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s):  a) For the facility at 16631 Scheer Blvd., Hudson, FL 34667, as observed on or about 04/29/2015, The forklift used on site was defective/damaged and unsafe to operate in that its right rear tire had missing chunks of rubber up to an inch of rubber lost, its horn was not functional, and its rear left taillight was broken.

29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing and labeling.  For the facility at 16631 Scheer Blvd., Hudson, FL 34667, as observed on or about 04/29/2015:   a) portable power strips were used to power industrial equipment such as a drill press, a bench grinder, a belt sander, and fans. The portable power strip was also hard wired into a junction box. This is not the intended use of the power strip and is a potential fire hazard.  b) Extension cords were hard wired into lighting fixtures. This is not the intended use of these flexible cords.
