29 CFR 1926.1408(a)(2): The employer did not determine if any part of the equipment, load line or load (including rigging and lifting accessories), if operated up to the equipment's maximum working radius in the work zone, could get closer than 20 feet to a power line. On or about April 27, 2015, during the construction of a restaurant at a worksite located at 1056 East River Rd., Cleveland, Ohio, structural steel erection activities required equipment and materials to be within 20 feet of energized 11kv power lines. The employer did not ensure that employees were protected from electrical shock/electrocution hazards.

Employees were thereby exposed to electrical shock/electrocution hazard.
