29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: On or about September 15, 2025, during the setup and delivery of roofing insulation on a low-sloped roof of a commercial building located at 15600 Chagrin Blvd, Shaker Heights, OH 44120, the employer did not ensure employees were protected from falling through uncovered holes (skylights) in the roof by covers, personal fall arrest systems, or guard rail systems erected around uncovered holes (skylights). This resulted in an employee falling through an uncovered hole (skylight) approximately 17 feet to the concrete covered ground below.
